Contract To Lend
 

I agree to lend to the Kentucky Heritage Quilt Society my KHQS Challenge Quilt for the year June 2009–June 2010. The Challenge Quilts "CELEBRATE THE FEMININE" will be available to travel throughout the Commonwealth of Kentucky and to KHQS members by UPS shipping for display. My work may be photographed and used by KHQS in any promotion or report.
 

I understand that KHQS will not be responsible for damage or theft occurring during this year.
 

I am also enclosing, as required by KHQS, a brief description of the quilt (artist’s statement). This is in order for KHQS to print cards to be attached to each quilt whenever they are shown. These cards will be pinned to the quilts each time they are shown and then removed and packed with the quilts for shipping.
