ENTRY FORM

Entries should reflect theme – “Save the Planet.” Examples are quilts made from recycled materials (denim jeans, old dresses or shirts, ties – and remember our challenge for 2009 Get-Away); patterns reflecting resources that need preserving, such as Ocean Waves, Flower Garden, Field and Stream, Farmer’s Fields, Prairie Flower; patterns that reflect endangered species; other patterns, such as Waste Not, Want Not, Danger Signal, and Snowball.

Quilts will be returned in one of three ways-

1. Quilts may be picked up from co-chairmen between June 8 and June 11 at a pre-arranged time OR

2. Quilts will be transported by co-chairmen to Getaway and may be picked up there by entrant or designated representative (please specify designee here) __________________________________________________ OR

3. Quilts will be shipped to address indicated _________________________________________________
