I would like to register for the KHQS Design Seminar 2007, “ Free Motion Quilting, Designing with Cutouts, Sensuous Lines, and Going in Circles ,” to be taught by Carol Taylor, of Pittsford, New York, on September 24-27, 2007, at the Catherine Spalding Retreat and Conference Center, Nazareth, Kentucky.

I understand that spaces will be filled on a first-come, first-served basis. Additional names will be placed on a waiting list.

I further understand that a KHQS member attending the Seminar will owe a balance of $275 by August 1, 2007 ($325 for non-KHQS members). [Day rate balance: $175/members; $225/non-members]
